About the Treatment

What Is the Laser Bandage Protocol?

The Fotona dual-wavelength laser system combines two technologies already used elsewhere in your care: the Er:YAG laser to gently seal and decontaminate the lesion, and the Nd:YAG laser in photobiomodulation (PBM) mode to calm inflammation and accelerate the body’s own healing response.

Rather than just masking discomfort, this approach targets the lesion directly — reducing the bacterial and viral load at the site while stimulating the surrounding tissue to repair itself faster.

Why “bandage” is the right word: The Er:YAG pass creates a protective, sterile seal over the open lesion — similar to what a bandage does, but from the inside out. That seal reduces pain almost immediately and protects the site while the photobiomodulation step does the deeper healing work.

Unlike a filling or a cleaning, cold sore and ulcer treatment doesn’t map to a standard covered dental procedure, and most insurers don’t reimburse specifically for the use of a laser. This is billed as a stand-alone service. In keeping with Dr. Albadran’s biological, drug-free philosophy, the fee reflects a chemical- and medication-free approach to healing — no antiviral prescription, no numbing injection required for most patients (topical or local anesthesia remains available for comfort if needed). Laser and photobiomodulation therapy for cold sores (recurrent herpes labialis) is one of the more thoroughly studied applications of low-level laser therapy in dentistry, with a growing multi-decade research base.

A randomized controlled trial published in Photomedicine and Laser Surgery found that low-level laser therapy produced a clear effect on both initial healing and the length of the recurrence-free period between outbreaks, with no observed side effects.[1]

A 2018 systematic review (Lasers in Medical Science) evaluating six clinical trials found consistent support for laser therapy’s effectiveness in managing and reducing recurrence of herpes labialis, compared to standard care.[2]

A 2024 randomized controlled trial (Clinical Oral Investigations) found that photobiomodulation therapy reduced pain significantly faster than acyclovir cream — the standard antiviral treatment — reinforcing the pain-relief benefit specifically, even where healing-time results vary study to study.[3]

Cold sores respond best to laser treatment at the very first sign of an outbreak — the tingling, itching, or burning sensation that precedes a visible lesion (the prodromal stage). The sooner treatment begins, the shorter and milder the outbreak tends to be.

  • Early vesicle stage — still highly responsive to laser treatment
  • Crusted or later-stage lesions — can still benefit, though results are typically less dramatic
